SIPA Application

I've included Jackie, my coordinator, on this email.  She can help answer specific questions you might have.

Overview:

[Specialty] Our process is really simple.  To join SIPA, we use CAQH.  You may already use CAQH for other payors.  Please be sure your CAQH profile is up-to-date.  The major plans we participate in are Aetna, Sentara, MedCost, and the Valley Health Employee Plan.  The application fee is $100, and $1000 membership for 3 years ($3100 total over 3 years).  The $3000 will be converted into two SIPA shares.
